How we collect information

Most information collected by us is provided by you, either from this site or from phone calls, emails, or faxes with our office.  We also may obtain information from public information, such as state medical board associations.  Information from prior carriers may be obtain, after we receive your consent to do so.

We may disclose any information which we believe necessary to conduct our business as permitted by law.  This disclosure may include information we receive from you on applications or other forms provided to us, such as names, addresses, social security numbers, birth date, employer information, etc.  This also includes information regarding policy coverages, claims histories, license actions, etc.

We may, as permitted by law, disclose your information to nonaffiliated third parties, such as insurance brokers, insurance companies or finance service providers, as necessary to conduct our business.  We do not disclose information to other companies or organizations not affiliated with our course of business for the purpose of using information to sell their products or services to you.
